Immigration Physicals  

The Department of Homeland Security has   -
revised it's requirements.

 
 

All physicals are done by appointment only. Immigration medical forms may be picked up 48 hours after the initial visit except for those physicals performed on Friday or the weekend.  Pricing & details click here


NEW! All applicants must present written documentation of the following:

·      Physical Exam performed by a Civil Surgeon authorized by the Dept. of Homeland Security.

·      Have a Tuberculin test (all patients older than 2 yrs.)

·      HIV test ( 15 years or older)

·        Test for syphilis ( 15 years or older)

·      Current Immunizations
 

If a tuberculin test is positive, a chest x-ray & refer all to a specialist or the County Health Dept. will be necessary.

All applicants coming to the doctor for their physical and other necessary testing must bring medical documentation of all vaccines they have received in the past. If such documentation is not available the patient will need to obtain all the required vaccinations. If you have received live virus vaccines such as measles, mumps, rubella or varicell (chickenpox) within 30 days of your visit for your physical please let your doctor know. This may prevent you from being able to obtain the tuberculin test on your first visit.

PLEASE BRING THE FOLLOWING INFORMATION:

Picture Identification

·      All  medical forms will be filled out with the name that appears on the picture identification (ID) unless 

·        A female that has recently married brings her marriage certificate and requests that the medical form is filled out in her married name.

Immunization Records

·    All records of Measles, Mumps and Rubella, Tetanus and Diphtheria, Chicken Pox (1st and 2nd dose).

·         If 65 years or age or older, proof of influenza and pneumonia vaccine or the vaccine will be given during the physical (This only applies during flu season (October 1 - February 28)

Translator if Needed

·        If medical records are not in English please have them translated before presenting for Medical visit.

Our Approved Physician Provider is Spanish speaking and we have several bilingual staff members.
